EXHIBITOR INFORMATION and REGISTRATION
Thank you very much for your support of this year's Fred Miyazaki Kaiser Permanente National Pelvic Floor Disorders Symposium at The Lodge at Sonoma - A Renaissance Resort & Spa in Sonoma, California on Friday August 7, 2009.

To both register for the conference and make a hotel reservation, select the Exhibitor Registration option at the bottom of this page. Please note a separate Exhibitor Registration should be completed for each representative of your company who will be attending the conference.
A wine and cheese reception for attendees will be held on Thursday, August 6th at 7:00 pm at the hotel. The cost is $50 per person to attend. If you wish to purchase a ticket, select the Wine and Cheese Reception option at the bottom of this page after you have completed your Exhibitor Registration.

EXHIBIT HALL
Location: Stone Building
Setup Time: Thursday, August 6th after 3:00 PM. Signs will indicate the location of your table and we will have all shipped materials delivered to this room.
Exhibit: We will provide 1-6' skirted table, 2 chairs and wastebasket for your exhibit. Payment for any special services required should be arranged with the Conference Services Manager at the hotel as services are ordered.
